Is it possible to use signal 74391 with Märklin
breakmodule if i vit of the plug?

Anything is possible with some work and design changes.
The 74391 is a start-up signal meaning it is a set of LEDs with resistors and a cable / connector to plug to a signal control box 72751 or an M84

The breakmodule 72442 is a relay with some circuitry (diodes) to provide a slowing-down zone.
The breakmodule must be controlled by a digital decoder or by a switch controller
The breakmodule has a spare SPDT marked as signal to drive a luminous signal such as the 74391 but you must to a few things to connect
Here is an example of a breakmodule use

Making the adaptation of the 74391 signal to the breakmodule
- from the signal must come 3 wires to connect to a powered SPDT (like an M84 decoder)
- find a veroboard (or equivalent)of the appropriate thickness to cut and plug into the connector and wire to the spare SPDT of the breakmodule installing in series a resistor (1 kOhm and a DC power source (+5 or +12VDC)

Sorry for not being more precise as I never did it myself, so a bit of testing is in order
