On 15th January 2014 a NS intercity derailed near the station of Hilversum,Holland.
The investigations showed that the derailment was caused by a broken clip of the switching mechanism.
The clip was broken because of material fatigue.
The fatigue was caused by repeated opening of the switch tongues ("wissel tongen") by moving trains,putting strain on the mechanism.
In addition,regular maintenance of the track lacked attention to the switching mechanism so that wear and tear would have been noted in time.

If you are at a railway crossing on your bike and the train crashes in front of you. Don't waste time trying to pick up the bike and turn it around.. Drop the bike and RUN! RollEyes

You have to wonder how the proposed driver-less trains would respond to this type of event. I suspect that they would be too stupid to realise that anything had happened as the loco is still on the correct line, and continue on to take out the on-coming train..

Adrian, it would be the fault of the on-coming train but if this train was also driverless, 200 people have been killed and no one can be charged for the accident. 2000 people have been killed by a computerized machine gun which went off accidentally, 20 people got killed on a freeway as a driverless car plunged into other stationary cars, the police were unable to interview the driver.

as we all know when we were young we were responsible for our own actions but since than its every bodies else fault and we know if you are under the influence of alcohol or drugs or you were bought up by parents it will never be your fault
